Description
Documents should not be stored by extension as the latter can
change !
Use a simpler hash/hash/image... based on version id.
By the way: do not store calculated images (bird_pv.jpg) inside
the version's folder because it makes it very hard to destroy
calculated images on iformat change. (see #120 ImageBuilder formats per site).

fixes #194 multiversion bug: cannot publish 'replaced' version from other author.
