[add] not working in table context

This bug happens because forms have to be inside a 'td' or outside the 'table', not between 'tr' and 'td'. We could detect that we are in a table and put the form tag inside the 'td' but then would not work.
Better not fix at all and propose this syntax where a form is explicitly defined:
status changed from new to closed.
resolution set to wontfix.
summary changed from r_add not working in table context to [add] not working in table context.
